Impact of Fracture Liaison Service in managing fragility fracture at tertiary hospital center, Malaysia

Abstract
Introduction: Osteoporosis is defined as deterioration in bone and architecture resulting in weakness of the bone. The risk for subsequent fragility fracture is 3 times higher for those who sustained previous fracture. The awareness and rate of osteoporosis management are low among Malaysia hospitals, which jeopardize the patient's risk of secondary fracture. The aim of the study to identify the efficacy of Fracture Liaison Services (FLS) in improving overall osteoporosis management among patient in tertiary teaching hospital, Malaysia.

Methods: This is a prospective study in comparison with data in 2016. Fracture Liaison Service was started in August 2018. Patient with fragility fracture reviewed in a university teaching hospital were identified from August 2018 until April 2019. The demographic data, diagnosis, assessment and management of osteoporosis and diagnosis were recorded.

Results: There were 220 patients (57, 25.9% male and 163, 74.1% female) identified. The mean age was 75.0 years old in comparison with 73.8 years old in 2016. With FLS, the percentage of patient having bone mineral density (BMD) assessment has increased from 18.6% to 54.5%. There is significant improvement of patients getting the anti-osteoporotic treatment from 16% to 62% whereas the calcium and vitamin D prescription have also improved by 10% compared to previous result.

Conclusion: FLS is proven to be effective in improving osteoporosis care gap. We suggest that FLS should be implemented in most hospital to improve osteoporosis management and reduce the risk of secondary fracture.
